Step 1: Get an Adding Form from the Records Section (Room _)
Step 2: Fill up the form completely and have it signed by the cluster head of your department.
Step 3: Go to the Admissions Office and choose from the available slots for the subject you need to change.
Step 4: An Enrollment Staff will encode the desired subject, section, and time. Once encoded, proceed to the Cashier for validation.
Step 5: Pay ₱10 for the Validation Fee. Once paid, it should already be reflected in your Online Registration Form.
Note: Make sure you'll pay the Php 10.00 at the treasury office to validate your adding of the subject.
Requests for TOR may be done every Mondays and Tuesdays.
1. Prepare the following requirements:
a. pc Graduation Picture (without cap)
b. pcs Documentary Stamps (available at all municipal/city offices)
c. completed Clearance Form
2. Get a Clearance Form from the Records Section (Room 1114) and fill up the form completely.
3. Have the clearance form signed by the Record Section and the Treasury Office
4. Proceed to the Requisition Area (Room 1101) to submit the Clearance Form and other requirements to file for a formal request.
They can enroll as long as they have claim stub from the school last attended so that the registrar will have the proof that they are requesting and for release.
If the student cant claim asap, they will be given a promisory and due date when they need to submit the official document.
In the event the student did not submit on the specific timeline, the student will not be able to enroll on the succeeding semesters.
Applicants with incomplete applications will be required to comply by a specific date
College / SHS Diplomas are required to have an SO prior to release. These SOs are issued directly by DepEd and CHED Regional Office. We will announce the availability of the diplomas as soon as we receive the SOs from DepEd and CHED.
While waiting for the official diploma, you may request for a Certificate of Graduation and/or Certificate of Grades anytime after graduation.
