ON THE JOB TRAINING DEPLOYMENT PROCESS


 1. ORIENTATION
All students must attend an orientation scheduled by the Office of Internship Training & Job Placement. Please ensure you are properly groomed and dressed in formal attire.
After the orientation, you must obtain a referral form for your medical and psychological examination from the Health Services Office. You will have three (3) to seven (7) days to complete the initial requirements, which will be placed in a long, labeled brown envelope.


 2. SUBMISSION OF REQUIREMENTS
Students are expected to submit the following documentation:
*1 copy of Resume with a 2x2 ID picture (white background)
* Photocopy of your Registration Form
* Notarized Written Parent or Guardian Con
* Notarized Written Parent or Guardian Con
* Notarized Written Parent or Guardian Consent Form
* Photocopy ID of Parent/Guardian (with 3 signatures)
* Photocopy of Medical and Psychological Results (HSO/Guidance and ITJPO will determine if you can proceed based on the findings)
* Completed Endorsement Letter Recipients Form (I LEARN)
Please note that you have a minimum of 3 days and a maximum of 7 days after attending the orientation to complete these requirements.


3. INITIAL INTERVIEW
You will be scheduled for an initial interview prior to deployment. After passing the interview, your endorsement letter will be processed.
The Memorandum of Agreement Annex – A and Acceptance Letter can be downloaded from I LEARN.


4. BEGINNING OF OJT DUTY
After successfully completing the interview and submitting your requirements, you may begin your OJT at the designated company.
In determining visit or reporting hours to the ITJPO, for example, if you are required to complete 360 hours, you will report to ITJPO every 90 hours (360 hours ÷ 4).
During these visits, please submit documents such as:
* Evaluation Forms
* Learning Outcomes (I LEARN)
* Photocopies of your Time Card
* Other required documents
Note that you have a three-day grace period after the completion of your reporting hours to submit these documents.
Trainees will be asked by the ITJPO personnel if they have any issues or positive experiences encountered during the OJT.


5. POST INTERNSHIP REQUIREMENTS
Upon completion of your OJT hours, you must submit your Post Internship Training requirements for grading, as outlined in your trainee's checklist.
The Exit Interview Form can be downloaded from I LEARN and must be fully accomplished.
