Step-by-Step Procedure regarding the Incident Problem of Students with regards to Bullying and different Violation on the School Policies



First, the student filed a formal written complaint against a student/faculty/staff at the Offie of the student Affairs. The formal report shall be in writing and shall state the nature and the circumstances of the offense. Any evidence or other materials relevant to the case shall be included in the report.


Second, The OSA reviews all reported cases and forward cases possibly involving major violations to the Committee on Discipline for hearing and deliberation.


Third, the OSA shall notify the student concerned through a Complaint Notice. A copy of formal written complaint shall be attached to the Complaint Notice.


Fourth, the student shall be required to submit a written statement as an answer to the complaint including other relevant materials to support his/her statement.


Fifth, the student shall be given one (1) week starting on the date of receipt of the complaint Notice to prepared such required materials. Failure of the student to submit such relevant. materials shall be taken to mean that he/she chooses to waive his/her rights to present the case as requested.


Sixth, the OSA shall then interview the parties involved, conduct additional investigation necessary to ensure full confirmation of the facts.


Seventh, the OSA shall assess the gravity of the offense using evidence gathered and decide whether the case is minor or major. If the case involves a possible major violation, it shall be forwarded to the Committee on Discipline, shall include formal charges, the facts of the case, the evidence and other relevant materials presented, as well as the review of precedent case.


Eight, the Committee on Discipline, though OSA shall request the presence of the parties at the hearing deemed necessary.


Ninth, The OSA shall notify the parentsof student charged before the Committee on Discipline Tenth, the Committee shall convene on the set date of hearing. Each (party shall present his/her testimony, evidence, supporting document and witnesses. The Committee may question the parties and witnesses. It shall deliberate on the case and determine whether the student is guilty of the offense as charged.


Tenth, The Committee on Discipline shall thereafter submit its findings and recommendatiobn in writing to the OSA.


Eleventh, The OSA approve/disapprove the recommended sanctions and recommends sanction to the Board of Trustees. The Board of Trustees approves/disapproves recommended sanction for school major cases.


Twelfth, the student may appeal to the Board of Trustees and they have the authority to make the final decision on the case. The College Dean is notified and is requested to implement the sanction and also notified the parents to sign the notice of student violation...
