TRANSFER CREDENTIAL REQUEST



All student requesting a Transfer Credential Request must have no financial, documentary and academic obligation.


Requirements:



. Endorsement Form from Office of Student Affairs (OSA)
. Consent from Guardian (3 pcs)
. LATEST Blue Registration Form (3 pcs)


If the representative will process the documents:



. Authorization Letter from student (1pc)
. Photocopied ID of the Requestor and Representative (1pc each)


Step 1: Clearance
The student must secure a clearance from the Records Section Room 1114. The designated staff will interview the student for the updating of the information and other necessary process.


Step 2: Signing of Clearance
Clearance must be signed by the Office of Student Affairs, Collecting Office and Registrar's Office. The Office of Student Affairs will release an Endorsement letter approved and signed by the OSA Coordinator.
The student may need to photocopy 3 pieces of their Consent from Guardian and LATEST Blue Registration Form before proceeding in the Records Section.


Step 3: Signing in Office of the Registrar
The student will proceed in the Records Section for the Signing of Clearance for the Office of the Registrar. They will need to provide the Endorsement Letter, Clearance, Consent Letter and Registration along with the photocopies. The Office of the Registrar will consolidate the papers before proceeding in the Requisition Area.


Step 4: Filing of Request
The student will file a request in Room 1101 (Counter 1). They must present all the requirements. They will need to fill out the Official Request of Documents.


Step 5: Payment
The students will present the Official Request of Documents to the Collecting Office After the Collecting Office Room 1111-1110 verified the information, student must proceed with the payment.
The Official Receipt released by the Collecting Office must be photocopied before proceeding back in the Requisition Area.


Step 6: Claim Stub
The student must present all the documents from the Collecting Office. The Designated Staff will set a Due Date for the requestor when they will be able to claim the document.
